いつもAQUA UMEDAをご利用いただきありがとうございます。

来月より新年度となるにあたり、
万が一の怪我に備え、会員の皆さんにスポーツ保険ご加入をお願いしております。
お申込みは受付にて承ります。
（基本的に会員全員の方にご加入いただいております）

被保険期間：2021年4月～2022年3月末
保険料　　：2000円

なお、登録が月末になりますので申請の翌月より
保険適用になります。
ご理解いただきますようお願い申し上げます。
